Osgood Schlatter syndrome presents in children who are experiencing growth spurts, (boys, 12-15 years; girls, 8-12 years) with local pain, swelling and tenderness over the patella tendon (tibial tuberosity). It caused by overuse and symptoms are exacerbated with sporting activities that involve jumping (basketball, volleyball, running) and/or on direct contact (e.g. kneeling).

It normally presents as a painful lump under the kneecap. You may notice that the bone is more prominent than the less painful knee. The pain in the knee may appear after sport and dissipate while running in the early stages. As the condition progresses, pain may come on while running and eventually will last between training sessions and playing sport will become particularly challenging.

Treatment

Treatment includes relative rest and activity modification from the offending activity as guided by the level of pain. Activity modification requires an analysis of the current activity levels and a reduction of total load to minimize knee pain and swelling. Lower limb muscles strengthening along with specific exercises to address movement patterns that may be creating overload at the knee joint is vital. This requires a tailored rehabilitation programme based on initial assessment and identification of potential compensation in their movement patterns.
